Mostly to get a firm, positive seat. Also because the cotton pellets the endodontist placed in the access openings came out while I was prepping.
Beautiful!! Great idea using the Lava inlays as margin elevators. Karen you can always fill in the access hole with the Replace tool.

Ross great work as usual. Never really thought about using Lava to elevate the margin in a case like this. Any concern with longevity vs burying the emax margin down where the Lava is?

No concerns for me, John. I would hate to have to go after those margins a second time, plus I feel more confident on less-than-ideal preps with LU than I do with e.max because of milling concerns. It's really a concept stolen from Pascal Magne, who regularly uses direct composite for margin elevation.
Great showcase of your talents and of the technique Ross. What happened to need endo on (both) teeth? It looks like you were clear of the nerves in the Lava Ultimate post-op pics.
Minimally retentive preps NEED some kind of anti-rotation/positive seating mechanism or you will have a nightmare getting them seated. The endo access created that nicely for you.

Ernie, we gave them a chance, but both teeth developed symptoms and required endo after the onlays.

I was wondering about the need for endo myself. Thanks for clearing that up. Would you have left them as onlays if there were no symptoms?

How did you prepare the lava ultimate surface for bonding w eMax? Did you have to use sand blaster? Can you go over the steps used to prepare the different surfaces? Nicely done. How long did it take for the tooth to be symptomatic ? Was the tooth necrotic when you opened it?
